It looks like you fixed issues 1 and 4, but 2, 3, and 5 are still issues.
Ok, a few issues:
1. No filter modulation: -3
2. Panning on all oscillators set to the right?: -3
3. Phase modulation?: -2
4. Depth on modulators too low to be heard: -4
5. insert effects not connected: -4
34/50 = 68% D
