It really depends on genre. For EDM styles, riser effects are common to signal the end of a section. More traditionally, you'd use percussion like tom fills to do the same. You can complement that with the opposite effect (like a crash) at the start of the next section. Smoothing transitions usually means to have an instrument (like a lead bassline or pad) playing uninterrupted through the transition. It really depends on each track.
